Health Information

Prior to Kindergarten

• Prior to the first day of kindergarten, the following immunizations are required: 
  • DtaP -5 doses
  • Polio - 4 doses
  • MMR- 2 doses
  • Hepatitis B- 3 doses
  • Varicella - 2 doses
• A Dental Screening Form must be completed by a dentist, dental hygienist, or trained medical staff. 
• A vision screening is required by Iowa law and will be completed at school or can be completed within the health physical.

3rd Grade

• A vision screening is required by Iowa law and will be completed at school. Download the Vision Screening Form on this page.  

Prior to 7th Grade

• Prior to the first day of 7th Grade, the following immunizations are required:
  • Meningococcal vaccine
  • Tdap booster

9th Grade

• A Dental Screening Form must be completed by a dentist, dental hygienist, or trained medical staff.  Download the Dental Screening Form on this page.  

Prior to 12th Grade

• Prior to the first day of 12th Grade, the following immunizations are required:
  • Meningococcal vaccine

7-12 Grade Activities

The Iowa High School Athletic Association requires students who are participating in sports to have a current health physical. It must be signed by the healthcare provider and parents. Physicals are valid for 13 months. A Physical Form can be found HERE. 

Please be sure to turn in proof of these requirements or a valid Certificate of Immunization Exemption to Ashten Pottebaum by the first day of school.
